It is time to update the school library card issued to your school by the Des Plaines Public Library.
Please return one signed copy of this letter along with one piece of photo identification such as a photocopy of his/her driver’s license or state identification card for the person who will keep the library card and is designated as the school representative.
Upon receiving the current information, we will update your school’s record in our system and mail a new card to the person you designate. The school is responsible for all items checked out on the card. The library card is only for use at the Des Plaines Public Library for school-related materials. It is valid for the school year and cannot be photocopied; however, the library card number may be put on a smartphone (see http://dppl.org/patron-services/scan-your-card for more information.)
If you would like to authorize more than one individual to use the card, please attach a list of names. All authorized staff will need to present the school library card or have it on a smartphone to checkout. If the school library card should become lost at any time, please notify the library immediately.
