TELEPHONE ASSISTANCE

Individuals with income tax-related questions can call the IRS:  1-800-829-1040 (Monday-Friday, 7 a.m.-7 p.m.) and its business counterpart, 1-800-829-4933 (Monday-Friday, same hours). To order forms, call:  1-800-829-3676.

The State of Illinois offers the following numbers to individuals with income tax-related questions: 1-800-732-8866 (toll-free) or 217-782-3336 (Monday-Friday).

IRS

The IRS office closest to the Des Plaines Public Library is at 5100 River Road in Schiller Park, about five miles south of downtown Des Plaines. Phone: 847-737-6688. No live telephone assistance. To make an appointment, call 844-545-5640.

TAX FORMS

** NOTICE: WE DO NOT CURRENTLY HAVE TAX FORMS. We expect them to arrive at the end of January or beginning of February 2025. **

Again, we DO NOT currently have the federal 1040 and 1040-SR booklet for 2024, as well as its separate instruction booklet. However, when they arrive they will be available on the north end of the first-floor lobby or the income tax forms area on the fourth floor. Also, you will be able to find the federal 1040 and 1040-SR booklet for 2024 in Spanish in both library locations. When we get them in we will also have the Illinois 1040 & the instructions available in both locations.

We are expecting the forms listed below. Please call 847-376-2841 to see which ones are in stock.

Other expected state forms:

-- Schedule ICR (Illinois Credits) + separate instructions

-- Schedule IL-E/EIC (Illinois Exemption and Earned Income Credit) + separate instructions

-- Schedule IL-WIT (Illinois Income Tax Withheld) + separate instructions

-- W-3 (Transmittal of Wage and Tax Statements)
